Dr. Peter Levins, MD

Dr. Peter Levins, MD is Board Certified in Family Medicine and has been practicing in Arizona since 2000. Dr. Levins joined Tatum Highlands Medical Associates in 2013 as the owner and Medical Director.


Dr. Levins is from Southern California where he attended Loyola Marymount University. Dr. Levins graduated Magna Cum Laude from Loyola Marymount University, before attending the Medical College of Wisconsin for medical school. During medical school is when he developed his appreciation for Family Medicine and the Medical College of Wisconsin provided varied experiences that led him to pursue Family Medicine as his specialty.


After medical school, Dr. Levins relocated to Phoenix for his residency program at Phoenix Baptist Hospital, where he served as the Chief Resident prior to completing his residency in 2000. Upon graduation, he practiced medicine at Union Hills Family Medicine before moving his practice to Scottsdale. Dr. Levins was a partner at Scottsdale Family Health for 12 years, where he provided full spectrum Family Medicine to his patients in both the outpatient clinic and in the hospital setting as well.  In addition to his group practice, Dr. Levins worked for 10 years as an instructor and community faculty member for Scottsdale Healthcare’s Family Medicine Residency program and as faculty for the University of Arizona Medical School in Phoenix. In these roles, Dr. Levins supervised residents in their pursuit of their medical degree.


Dr. Levins is married to his high school sweetheart, and they have raised two sons, who are graduates of Arizona State University. They enjoy hiking and biking, all things sports related, gardening, and travel. They have grown to love the Valley of the Sun and look forward to many more years in Phoenix. Dr. Levins enjoys all areas of Family Medicine, but has a special interest in Geriatrics, Preventative Health, and Pediatrics. He looks forward to growing his practice and continuing to serve the healthcare needs of this community.

Cari Montoya, FNP-C, MSN

Cari Montoya is Board Certified in Family Practice through the American Academy of Nurse Practitioners. She is a graduate of both Arizona State University Grand Canyon University, and has been providing healthcare in the valley since 2010.


Cari obtained a bachelor’s degree in Family and Human Development from Arizona State University in 2010, and a second degree when she obtained a Bachelor of Science in Nursing from Grand Canyon University in 2012. After graduating, Cari worked for Honor Health, formerly known as Scottsdale Healthcare, for 8 years where she provided direct patient care and even took the time to become a certified medical-surgical nurse.


Cari’s desire to further her education led her back to Grand Canyon University, where she graduated with her Master of Nursing degree and became a certified Family Nurse Practitioner.  Cari considers herself a life-long learner, therefore she continues to educate herself by attending continuing education courses and staying current on relevant medical research.


Cari is a native Arizonan and is proud to call Phoenix home. She enjoys spending time with her husband, her growing family, and their two dogs.  She enjoys traveling, hiking the Arizona trails, exploring local coffee shops and restaurants, and running her online boutique.


Cari has a special interest in wellness, preventative healthcare, and women’s health. Warner Coelho, FNP-C, MSN

Warner Coelho, FNP is Board Certified in Family Practice through the American Association of Nurse Practitioners. Warner’s passion for healthcare led him to obtain his Associate of Nursing in 2012. In 2016, he returned to school and completed his Bachelor of Science in Nursing from Grand Canyon University. He later attended Chamberlain University where he obtained his Master of Nursing degree and graduated with honors.


Since 2012, Warner has been providing direct patient care in the medical field. His first 8 years in nursing were spent at St Joseph’s Hospital and Medical Center. During this time, Warner’s care was focused in the management of acute and chronic disease processes. Warner has gained extensive experience in the specialties of Cardiology and Pulmonology with a focus on lung transplant and advanced heart failure patients.


Warner has lived in Arizona for most of his life and resides in Northeast Phoenix with his wife and children.  In his free time, Warner enjoys spending time outdoors, camping, hiking, and bicycling. He also enjoys travelling and exploring the world with his family.


Warner’s dedication to the promotion of health and wellness is what drove him to become a Family Nurse Practitioner. Warner has special interest in men’s health, pediatrics, and disease prevention.


Warner joined Tatum Highlands Medical Associates in 2020 and looks forward to growing his practice in the community he has called home for most of his life.
